Include Rx with all items in case box then proceed to STEP 3

If you are shipping your cases to us:
Put as many cases that fit in each box (separated by a ziplock style bag)

If using a shipping mailer bag
(FEDEX / USP) put as many boxes in one as possible

Attach preprinted shipping label
(call our office for shipping label needs or use form below to request labels to be sent by email/fax)
